1. Develop and follow clear cut procedures
for supervision including a single point of contact for scheduling families
and direct care providers (Prior and current reviews) |
A letter will be sent to all consumers/families identifying the Community Staff Supervisor (CSS) as the primary contact for scheduling needs. New families will be provided this information at the time services are initiated. Staff will be sent an internal memo clarifying the CSS as their direct supervisor. |

2. To minimize delays, provide assistance to families with tracking and advocating for needed equipment. |
A tracking form will be attached to each waiver with Specialized Medical Equipment. It will include date ordered, vendor name, contact, date shipped, date delivered and other pertinent information. A comment section will allow for description of delays and the reason. This form will be available for parent inspection and will be placed in the record when completed. |

3. Develop and implement a plan for the minimization of delays in processing plan of care renewals. |
A tracking sheet currently exists to monitor Plan of Care (POC) amendments as drafts are received, reviewed, sent for costing, cost sheet return date, date returned to Care Coordinator. This is signed by both Hope staff and Care Coordinators. This system functions when it is observed. Efforts will be directed at ensuring compliance. |

4. Develop and implement a plan to enhance your efforts to assure high quality in all services. |
Ongoing efforts are being made to quality assure services are maintained. These include documentation, tracking, record keeping, QA reviews, service utilization reports, consumer satisfaction surveys and other tools to evaluate quality of services. |

5. Develop and implement a policy for
advance planning of services and related staffing needs including planning
for summer after the close of school. |

6. Reactivate the local advisory board with
a goal of including direct consumers (Standard #6) |
Board members will be recruited to include direct consumers and the Community Resource Team will be functional in the advisory capacity for which it was intended. |

7. Develop
and implement a plan to assure that all staff have adequate training in basic
skills needed in working with people who experience disabilities before the assume their duties.

All personnel hired currently meeting the hiring requirements for employment with Hope Community Resources and receive basic training in agency standards, expectations, health and safety and procedures. Emphasis will be directed at providing training more specific to individuals with unique or complex needs. A staff retreat will be held annually to facilitate staff input on training needs. |

This action item is beyond the scope of the regional office. It is being addressed at the agency level through a variety of ideas and efforts to improve employee retention. These include pay incentives, a retirement program, educational incentives and other benefits. It is anticipated these incentives will be implemented as funding becomes available. |
