1.     Develop and institute a plan for adding direct consumers as members of the governing board. (Standard 6)

 

 

 

 

Hope primarily supports individuals who experience a developmental disability (cognitive).  Identifying an individual to sit on the Board of Directors as a full and equal participant has been challenging.  Over the next year, Hope will continue to search in two distinct ways:

 

1.                    Hope will build connectors with local community organizations affiliated with developmental disabilities and will seek referrals for potential candidates.

2.                    Hope will invite two individuals who receive supports from Hope and who volunteer to attend regular management meetings.  We believe that the concept of being on a Board of Directors is a concept not easily understood by many persons who experience a developmental disability (cognitive).  Even for those with the required skills and abilities, it is a difficult decision to make a commitment to meet the demands as a Board member.  It is hoped that by offering an experience associated with agency management meetings, Hope can mentor individuals in a self-determined exploration that could result in them asking to be a Board member.

2.     Evaluate the complexity of the needs of the region and the caseload and develop a plan to increase staffing.

 

 

 

 

 

 

 

The Program will review the existing office structure, program staffing levels, travel requirements, workload issues, and local supports available to the Bristol Bay Regional Program.  A plan will be developed to address any staffing deficiencies with identification of potential resources to address any additional staff needs.  Potential funding sources will be approached for assistance if unmet needs are identified.

3.     Formalize efforts to provide futures planning and employment opportunities. (Prior and current reviews)

 

 

 

 

 

 

 

The Program will continue to offer Personal Futures Planning as a support option to individuals receiving services.  Individual Support Plans will indicate whether or not an individual desires to have, or participate in the development of, a Personal Futures Plan on his/her behalf.  The Program will work with DVR and Hope’s Supported Employment Network in further exploring/developing vocational options for individuals with disabilities in the Bristol Bay Region.  All service options will be addressed in the person’s Individual Support Plan.

4.     Educate the community regarding how to access services and how to plan for future needs.  (Prior and current reviews)

 

The Program will provide quarterly inservice training to various community organizations and interested families on how to access developmental disability services and plan for future support needs.  The Bristol Bay Regional Office will maintain relevant brochures and information regarding service delivery and will work with other community organizations to provide a network of available information and resources for individuals experiencing a developmental disability.

5.     Develop a plan to increase community awareness and acceptance of people with disabilities.

 

 

 

The Program will work with the Community Resource Team in developing a plan to increase community awareness and acceptance of individuals who experience disabilities in the Bristol Bay Region.

6.     Seek further options for after-school programs for children who experience developmental disabilities.  (Prior and current reviews)

 

 

 

The Program will meet with local school personnel, community providers, and families to explore the various needs and family desires of disability related support options provided during after school timeframes.  The Bristol Bay Program will actively collaborate with other community providers to develop a plan to address community support options needed for children who experience disabilities.

7.        Develop a plan for deaf consumers to develop communication and social skills, including interaction with other hearing impaired individuals.

 

 

The Program will develop an Individualized Support Plan (ISP) for each individual receiving services in the Bristol Bay area.  Individuals who request assistance in addressing specific communication and social skills development will be provided support as indicated and outlined in his/her ISP.

8.     Investigate the potential for residential options for female individuals.

 

 

 

The Program will explore residential options and resources available in the Bristol Bay Region and generate a list of potential residential opportunities available to individuals.
