ILP will implement use of a narrative contact log to document all contacts, phone calls, and information.  Staff will be directed to sign all notations.  Home visit summaries will remain the same.

5.       Have parents sign and date that they have reviewed their rights with the program; this should be reviewed annually, usually at the time the IFSP is updated.  (Prior and current reviews)

Currently we rely on the physician, public health or audiology report as the second discipline.  This will be included in narrative report and copy of their report filed with the initial assessment

8.  Include revisions made on the IFSP’s.
